Frank Strada, head of Tennessee’s prison system, resigned after the failed execution of Christa Pike, who had been convicted of murder and sentenced to death. Governor Bill Lee said Strada offered his resignation and will leave later in the month; one report said he left on Saturday. An independent investigation is under way into what went wrong.
